By submitting the birthday form, you confirm that you have read and agree to these terms, including permission for Toast to the Weekend to include your first name in our monthly birthday shoutout unless you choose to opt out. ELIGIBILITY • Birthday celebrants of all ages may participate. • A parent or guardian must complete the form and manage participation for a child under 13. • A valid first name, email address, and birthday month and day are required. • Limit one enrollment and one birthday treat per person, per calendar year. • No purchase is required to participate. • Monthly registration or RSVP deadlines may apply. YOUR BIRTHDAY TOAST • Toast to the Weekend will designate one Birthday Celebration Day each month. • The Celebration Day, pickup location, pickup window, and any RSVP instructions will be provided to that month’s birthday celebrants. • Any required RSVP or pickup confirmation must be completed by the stated deadline. • Your complimentary French toast treat will be freshly prepared and must be picked up in person during the designated Celebration Day pickup window. • The treat expires at the end of that Celebration Day. • Treats cannot be picked up early, redeemed on another date, carried over to another month, shipped, delivered, exchanged, or redeemed for cash or store credit. • If you are unable to attend the designated Celebration Day, an alternate pickup date cannot be guaranteed. • If you register after your birthday month’s deadline or Celebration Day has passed, you may participate the following calendar year. • Treat flavors and selections are determined by Toast to the Weekend and may vary each month. Customizations and substitutions are not guaranteed. • Birthday treats are nontransferable and have no cash value. ALLERGIES • Please notify Toast to the Weekend of any allergies or dietary concerns before confirming your treat. • Products are prepared in a shared commercial kitchen that may come into contact with common allergens. • Allergy-friendly substitutions and accommodations cannot be guaranteed. MONTHLY BIRTHDAY SHOUTOUTS • By submitting the birthday form, you give Toast to the Weekend permission to include your first name in our monthly birthday social media post. • Last names, email addresses, birth years, and exact birth dates will not be shared. • If you prefer not to be included, email [email protected] before your birthday month. • Opting out of the social media shoutout will not affect your birthday treat. • A parent or guardian may opt a child out of the monthly birthday shoutout. COMMUNICATION AND PRIVACY • Signup information will be used to manage the birthday program and provide Celebration Day details. • Birthday signup does not automatically enroll you in general marketing emails unless you choose to subscribe separately. • Toast to the Weekend will not sell your personal information. • You may request removal from the birthday program at any time. Removal may prevent Toast to the Weekend from providing future birthday treats or pickup information. • Participants are responsible for providing a working email address and reviewing the Celebration Day information provided. PROGRAM CHANGES • Toast to the Weekend may update, pause, reschedule, or discontinue the birthday program when necessary. • Celebration Days may be changed due to scheduling, weather, ingredient availability, or other circumstances. • If Toast to the Weekend changes or cancels a scheduled Celebration Day, updated information will be provided to affected participants. • Duplicate, inaccurate, or fraudulent registrations may be removed.
